यह प्रोटोकॉल गर्भावस्था के दौरान सक्रिय रोग के साथ प्रस्तुत पेम्फिगॉइड गेस्टेशनिस को कवर करता है — ऐसे मामले जो प्रसवोत्तर या गर्भपात के बाद की अवधि में नहीं हैं।
नैदानिक परिदृश्य
गर्भावस्था के दौरान सक्रिय पेम्फिगॉइड गेस्टेशनिस जिसके लिए चिकित्सीय प्रबंधन की आवश्यकता है, जिसमें उपचार विकल्प गर्भवती रोगी के लिए विशिष्ट सुरक्षा विचारों द्वारा सूचित होते हैं।
उपचार दृष्टिकोण
प्रथम-पंक्ति प्रबंधन प्रणालीगत कॉर्टिकोस्टेरॉइड थेरेपी पर केंद्रित है। सामयिक कॉर्टिकोस्टेरॉइड और/या एंटीहिस्टामाइन को सहायक के रूप में जोड़ा जा सकता है। पूर्ण नियम — जिसमें एजेंट चयन, खुराक और नैदानिक मार्गदर्शन शामिल हैं — पूर्ण प्रोटोकॉल में उपलब्ध है।

- In refractory disease, systemic corticosteroids may be attempted.
- SCs (prednisone 0.5 mg/kg/day) ± TCs and/or antihistamines
- In case of failure/contraindication/side effects of systemic corticosteroids, we suggest to consider during pregnancy intravenous immunoglobulin therapy and, as second step, conventional immunosuppressants/immunomodulating agents such as dapsone or azathioprine, which are the last choice, which can be used safely during pregnancy as inferred from many studies involving patients with other diseases treated with these agents.
