Treatment of Herpes Zoster Ophthalmicus with Moderate to Severe Facial Pain

Clinical Scenario

This protocol applies to patients with herpes zoster ophthalmicus who are experiencing moderate to severe facial pain. The pain burden in this presentation is a key factor shaping the treatment strategy, warranting an approach that addresses both viral activity and inflammatory pain.

Treatment Approach

Management combines oral antiviral therapy with oral corticosteroids. The addition of oral corticosteroids is specifically indicated in this scenario: they have been shown to significantly shorten healing time and reduce symptoms in patients with moderate to severe pain.

Agent selection, specific options, and full treatment duration are detailed in the complete protocol.

References

Oral steroids (prednisolone) have been shown to significantly shorten healing time and reduce symptoms in patients with moderate to severe pain.

The standard dosage of acyclovir for herpes zoster infections in adults is 800mg five times a day (at approximately 4 hourly intervals).

Treatment should continue for 7-14 days.

The standard treatment regimen for valacyclovir is 1000 mg, three times per day for 7-14 days, and for famciclovir, it is 500 mg per day for 7 days.
