This protocol addresses herpes simplex virus (HSV) esophagitis in patients without impairment of immunity. The natural history and management approach in this population are distinct from presentations in immunocompromised individuals.
In patients without immune impairment, spontaneous resolution within 1 to 2 weeks is the expected outcome. Some patients may respond more quickly when treatment is initiated.
When intervention is warranted, a short course of antiviral therapy is the established approach — the complete structured regimen, including specific drug selection, dosing schedule, and duration, is available in the full protocol.

Spontaneous resolution usually occurs after 1 to 2 weeks in patients without impaired immunity, although some may respond more quickly if treated with a short course of oral acyclovir 200 mg five times a day or 400 mg three times a day for 1 to 2 weeks.
