Lumbar disc herniation occasionally presents with neurological findings that signal a more urgent clinical situation — specifically when severe lower limb muscle weakness or cauda equina syndrome is present. Prognosis in these patients is often poor without timely intervention, making the treatment approach distinct from routine disc herniation management.
This protocol applies when a patient with herniated lumbar disc meets at least one of the following presentations:

Immediate surgical removal of the herniated disc material is indicated when the patient demonstrates severe lower limb muscle weakness or when the patient has cauda equina syndrome (pain in both lower limbs, sensory disturbance, motor deficits, bladder and rectal disturbance, and sensory disturbance in the perineal region), since prognosis of these patients is often poor.
