Treatment of Hereditary Spinocerebellar Ataxia with Dysphagia and Unintentional Weight Loss

Clinical Scenario

This protocol covers patients with hereditary spinocerebellar ataxia who develop difficulty swallowing (dysphagia) leading to unintentional weight loss. Significant dysphagia can ultimately result in unintentional weight loss if patients struggle to maintain an adequate caloric intake.

Management Approach

When oral caloric intake can no longer be adequately sustained, enteral feeding support — including a gastrostomy-based approach — may be indicated to provide secure nutrition. The full clinical decision pathway, including precise indications and sequencing, is available in the complete protocol below.

If calorie intake cannot be maintained despite supplements, discuss the possibility of a percutaneous gastronomy (PEG) to provide secure feeding.
