Treatment of Hereditary Hemochromatosis in Decompensated Cirrhosis or Hepatocellular Carcinoma

Hereditary hemochromatosis (HH) complicated by decompensated cirrhosis or hepatocellular carcinoma (HCC) represents advanced hepatic disease and requires a distinct management approach beyond standard iron-depletion strategies.

Clinical Scenario

This protocol addresses patients with confirmed hereditary hemochromatosis who have developed decompensated cirrhosis or hepatocellular carcinoma (HCC). These complications indicate end-stage hepatic involvement and trigger evaluation for a definitive intervention pathway.

Treatment Approach

Current evidence-based guidelines recommend consideration of a transplantation-based intervention in this setting — the full eligibility criteria, referral thresholds, and structured clinical pathway are detailed in the complete regimen.

We recommend that LT be used in patients with HH who have decompensated cirrhosis or HCC (strong recommendation, low quality of evidence).

Referral for liver transplantation (LT) should be considered in patients with HH with end-stage liver disease or HCC.
