Treatment of Hepatocellular Carcinoma in Well-Compensated Cirrhosis (Child-Turcotte-Pugh Class A)

Clinical Scenario

This protocol addresses hepatocellular carcinoma arising in the setting of cirrhosis classified as Child-Turcotte-Pugh (CTP) class A — well-compensated hepatic function. The scenario is further defined by limited hepatic tumor burden, absence of clinically significant portal hypertension, and an adequate future liver remnant.

Key Conditions Present

Liver Cirrhosis — CTP Class A (Well Compensated)

Treatment Approach — Partial Overview

In this well-defined clinical situation, the management strategy centres on a surgical intervention targeting the hepatocellular carcinoma. The specific operative technique is selected based on individual patient factors. Full regimen details — including approach selection criteria and perioperative considerations — are available in the complete protocol.

In patients with cirrhosis, surgical resection should be considered the treatment of choice for patients with limited tumor burden, well-compensated cirrhosis without clinically significant portal hypertension, and an adequate FLR (Level 2, Strong Recommendation).

Minimally invasive liver resection (laparoscopic and robotic) may be performed to enhance recovery and lower risk of perioperative morbidity in selected patients (Level 3, Weak Recommendation).
