When a simple hepatic cyst becomes symptomatic, watchful waiting is no longer sufficient. The clinical question shifts to which volume-reducing intervention is most appropriate for the patient and the treating centre.
Symptomatic hepatic cysts can be treated surgically or with percutaneous aspiration sclerotherapy.
There are various methods for volume-reducing treatment, such as percutaneous drainage with or without sclerosing, minimally invasive deroofing, and complete resection. The most accessible and experienced method should be selected according to specific circumstances of each center.
