Treatment of Guillain-Barré Syndrome When Unable to Walk Unaided (GBS Disability Score Grade 3 or More)

In Guillain-Barré syndrome, the degree of functional impairment guides the decision to initiate treatment. Patients who can no longer walk without assistance—or who retain that ability but develop swallowing difficulties or autonomic disturbances—meet the threshold for prompt intervention.

Clinical Scenario

This protocol covers patients who are unable to walk unaided (GBS disability score grade 3 or more) within 4 weeks from onset of weakness. It also applies to patients still able to walk unaided but who present with swallowing difficulties or autonomic disturbances.

Treatment Approach

Immunotherapy is strongly recommended to be started as soon as possible for patients meeting this clinical threshold. In cases where a treatment-related fluctuation (TRF) occurs, repeating a course of immunotherapy may be considered—the specific options, eligibility criteria, and sequencing are detailed in the full protocol.
