Second-Line Treatment for Moderate-to-Severe Active Graves' Orbitopathy When First-Line Therapy Has Not Produced Sufficient Response

This protocol applies when moderate-to-severe, active Graves' orbitopathy — with a clinical activity score of ≥ 3/7 — persists after a full first-line immunosuppressive course and the required response benchmarks have not been met.

Clinical Scenario

Graves' orbitopathy classified as moderate-to-severe and active (CAS ≥ 3/7), with features that may include:

First-Line Treatment Did Not Work

Prior therapy: Intravenous methylprednisolone combined with mycophenolate sodium (or mycophenolate mofetil) for most patients, or intravenous methylprednisolone monotherapy in the more severe forms — together with antithyroid drugs to control hyperthyroidism.

Response targets not reached at 3 months (or by 6 months): At least a 2 mm reduction in lid aperture; at least a 1-point reduction in the 5-item clinical activity score; at least a 2 mm reduction in exophthalmos; at least an 8° gain in eye muscle duction; and improvement in at least two features in one eye without deterioration in the other.

Second-Line Approach

Following careful ophthalmic and biochemical evaluation, several distinct second-line alternatives are considered — including renewed intravenous corticosteroid-based regimens and other immunosuppressive or biologic options.
