Granuloma Inguinale: What to Do After Co-Trimoxazole or Doxycycline Has Not Worked
When first-line treatment for granuloma inguinale does not achieve complete healing of genital ulcers, a defined next-line protocol applies. This page describes the clinical scenario and the approach that follows.
First-Line Failure — Escalation Trigger
Treatment with co-trimoxazole or doxycycline was the initial approach. The goal — complete healing of the genital ulcers — was not reached, triggering escalation to the next treatment line.
Next-Line Approach & Goal
The next-line protocol adds an adjunct antibiotic to support healing in cases where lesions are slow to respond. The clinical goal remains complete healing of the genital ulcers. The full regimen, including selection criteria and structured options, is in the complete protocol.
References
- Gentamicin 1 mg/kg every 8 hours can also be used as an adjunct if lesions are slow to respond (Grade C, Level III)
- Duration of treatment should be for at least three weeks or until complete healing is achieved.
