Treatment of Resectable, Non-Metastatic Glucagonoma in the Body or Tail of the Pancreas

Clinical Scenario

This protocol addresses glucagonoma arising in the body or tail of the pancreas in patients with resectable, non-metastatic disease. Most glucagonomas are malignant and are located in the tail of the pancreas, often with regional lymph node involvement.

Treatment Approach

Management centers on surgical resection, preceded by structured preoperative medical preparation — the complete step-by-step protocol is detailed below.

References

  1. Most glucagonomas are malignant and calcified and located in the tail of the pancreas, with regional node involvement.
  2. The recommended treatment is distal pancreatectomy with splenectomy and resection of the peripancreatic lymph nodes.
  3. For patients with glucagonoma, treatment of hyperglycemia and diabetes is necessary, especially to control blood sugar level prior to surgery.
  4. All patients who might require splenectomy should receive preoperative vaccination against pneumococcus, haemophilus influenzae type b, and meningococcal group c.
  5. A hypercoagulable state has been reported in 10% to 33% of patients with glucagonoma. Therefore, perioperative anticoagulation can be considered because of the increased risk of pulmonary emboli.
