Treatment of Giant Cell Tumor of Bone of the Spine — Enneking Stage 2

This protocol addresses the management of giant cell tumor of bone (GCTB) arising in the spine when the tumor is classified as Enneking stage 2: symptomatic and intracompartmental (active).

Clinical scenario

The majority of GCTBs are symptomatic and intracompartmental (active; stage 2) or symptomatic and extracompartmental (aggressive; stage 3). This protocol applies to stage 2 spinal lesions — those that are symptomatic yet remain confined within their compartment.

Surgical approach — overview

For Enneking stage 2 spinal lesions, the recommended surgical strategy involves a subtotal resection of the tumor. The full procedural detail, eligibility criteria, and any additional considerations are available in the complete protocol.

The majority of GCTBs are symptomatic and intracompartmental (active; S2) or symptomatic and extracompartmental (aggressive; S3).

For Enneking stage 2 tumors, curettage is recommended.

Subtotal resection (i.e., curettage) and piecemeal total spondylectomy or total en bloc spondylectomy are suitable for the treatment of S2 and S3 lesions, respectively.
