Treatment of Active Gestational Pemphigoid in the Post-Partum or Post-Abortion Period
Clinical Scenario
This protocol addresses pemphigoid gestationis with active disease occurring in the post-partum or post-abortion period — that is, disease persisting or flaring after delivery or termination of pregnancy, not during pregnancy itself.
Management Approach
Active pemphigoid gestationis in this post-partum setting warrants a structured immunosuppressant or immunomodulating approach. - In the post-partum/post-abortion period, we suggest as first step treatment immunosuppressant/immunomodulating agents, including dapsone and azathioprine, leaving as second choices IVIg and rituximab due to pharmacoeconomic reasons.
