Treatment of Active Pemphigoid Gestationis in the Post-Partum or Post-Abortion Period

Pemphigoid gestationis does not always resolve with delivery or termination of pregnancy. When the disease remains active in the post-partum or post-abortion setting, the clinical approach differs from management during pregnancy itself.

Clinical scenario: Pemphigoid gestationis with active disease occurring in the post-partum or post-abortion period — not during an ongoing pregnancy.

First-line approach (partial): Management of this scenario centres on escalation of systemic corticosteroid therapy. The structured protocol defines the extent of that escalation and the considerations that follow — the full regimen is available via the link below.

  • In the post-partum/post-abortion period, we suggest as first step treatment immunosuppressant/immunomodulating agents, including dapsone and azathioprine, leaving as second choices IVIg and rituximab due to pharmacoeconomic reasons.
  • Increase SCs dosage (up to prednisone 1 mg/kg/day)
