Treatment of Pemphigoid Gestationis with Active Disease in the Post-Partum Period
Pemphigoid gestationis (PG) may persist or flare after delivery or following pregnancy loss. When active disease is present in the post-partum or post-abortion period, management differs from in-pregnancy care and a defined first-line approach applies.
Clinical scenario
Pemphigoid gestationis with active disease in the post-partum or post-abortion period — disease that has not remitted after the end of pregnancy.

- In the post-partum/post-abortion period, we suggest as first step treatment immunosuppressant/immunomodulating agents, including dapsone and azathioprine, leaving as second choices IVIg and rituximab due to pharmacoeconomic reasons.
- In refractory disease, systemic corticosteroids may be attempted.
- SCs (prednisone 0.5 mg/kg/day) ± TCs and/or antihistamines
