Treatment of Gastric Neuroendocrine Tumor in Poorly Differentiated Gastric Neuroendocrine Carcinoma (NEC)

This protocol applies to gastric neuroendocrine tumors classified as poorly differentiated neuroendocrine carcinoma (NEC) of the gastrointestinal tract — a distinct entity from well-differentiated neuroendocrine tumors (NETs).

Poorly differentiated NEC and well-differentiated NET are separate diagnostic categories. Accurate pathological classification is central to determining the appropriate treatment approach in this setting.

For nonresectable poorly differentiated gastric NEC, systemic platinum-based chemotherapy is the indicated approach. The specific regimen combinations and selection criteria are detailed in the full structured protocol.

The WHO Classification of Endocrine Organs (2017) and Digestive System (2019) categorizes NENs as well-differentiated NENs (termed ''NETs'') or poorly differentiated NECs.

In nonresectable cases, platinum-based chemotherapy is indicated, such as etoposide/cisplatin, irinotecan/cisplatin, and etoposide/carboplatin.
