Treatment of Superficial Extremity Frostbite (Grade 1–2)

This protocol covers superficial frostbite of the extremities at grades 1 and 2 — a range that includes presentations without cyanosis and those with cyanosis limited to the distal phalanx.

Clinical Scenario

Superficial extremity frostbite, grades 1 and 2, encompasses injuries at the superficial tissue level. Presentations range from superficial grade 1 injury without cyanosis to superficial grade 2 with cyanosis of the distal phalanx.

Treatment Approach

For adult patients with an expected inpatient stay exceeding 48 hours, intravenous iloprost infusion may be considered as part of the management approach. Admission to monitored care with continuous telemetry is required for the duration of infusion therapy. The complete protocol — including dosing approach, titration schedule, and duration — is available in the full regimen below.

  • Consider use of iloprost if patient is expected to have an inpatient stay longer than 48 hours.
  • Administer iloprost as a continuous intraVENOUS infusion for 6 hours each day for 5-8 consecutive days.
  • Admit to monitored care (continuous telemetry) for the duration of infusion therapy to monitor hemodynamics.
