Treatment of Transformed Follicular Lymphoma After Prior Anthracycline Chemotherapy
Histological transformation of follicular lymphoma to diffuse large B-cell lymphoma (tFL) represents a clinically distinct and high-risk situation. When transformation occurs in patients who have already received anthracycline-based chemotherapy, the treatment approach must account for prior exposure while considering fitness for intensive consolidation.
Clinical scenario
- Follicular lymphoma with histological transformation to diffuse large B-cell lymphoma (DLBCL)
- Previously treated with anthracycline-containing chemotherapy
- Patient is fit for high-dose therapy with autologous stem cell rescue
Treatment approach
Consolidation strategy
In eligible patients with tFL who have responded to salvage treatment, consolidation involving high-dose therapy with autologous stem cell rescue is the approach considered in this protocol.

References
- The treatment of patients with tFL previously exposed to anthracyclines with or without rituximab is varied and, in general, involves second-line regimens used for DLBCL in patients who are eligible for consolidation with HDT-ASCR.
- Offer second-line salvage chemotherapy regimens used for DLBCL in tFL who have received prior anthracycline and are fit for HDT-ASCR.
- Consider HDT-ASCR in tFL patients previously treated with R-chemotherapy, who respond to salvage therapy and are fit for HDT-ASCR.
