Treatment of Advanced-Stage Follicular Lymphoma with Low Tumor Burden and No Symptoms
Patients with follicular lymphoma who present at an advanced stage yet carry a low tumor burden and remain asymptomatic represent a distinct clinical scenario. The evidence-based approach for this group diverges significantly from higher-burden disease — and getting the initial management strategy right matters.
Clinical scenario: Follicular lymphoma, advanced stage — Lugano stage III or IV, or bulky localised disease — with low tumor burden and an asymptomatic presentation.
For asymptomatic patients in this staging category, current evidence supports beginning with active surveillance — a strategy of close clinical and laboratory monitoring without immediate systemic intervention.
