What Is the Treatment of Focal Nodular Hyperplasia When Intervention Is Needed?

Focal nodular hyperplasia (FNH) is a benign hepatic lesion that most commonly requires no intervention. However, in select exceptional cases — such as lesions that are pedunculated, expanding, or exophytic — active treatment becomes necessary.

When treatment is indicated, the decision between surgical and non-surgical management depends on the patient's fitness for an operative procedure. The structured protocol defines which patients meet criteria for intervention and specifies the preferred treatment pathway for each clinical situation.

The evidence-based approach centres on a primary surgical option — reserved for appropriate candidates — alongside an alternative pathway for patients who cannot undergo that intervention. The complete protocol specifies the criteria, sequencing, and conditions under which each path applies.

Treatment is only pursued in exceptional cases (e.g., pedunculated, expanding, exophytic) and resection is the treatment of choice.
Non-surgical treatments should be reserved for those unfit for resection.
