What Is the Treatment of Focal Epilepsy When Initial Monotherapy Is Unsuccessful?

Clinical Scenario

This protocol addresses patients with focal epilepsy whose focal seizures have not been adequately controlled on initial antiseizure monotherapy, and who require a structured approach to selecting the next treatment step.

Treatment Approach

When focal seizure control is not achieved with monotherapy, guidelines support the addition of a first-line add-on antiseizure medication chosen from a defined set of options. At least one of the recognised agents carries specific prescribing restrictions for women and girls of childbearing potential.

If the initial add-on choice does not achieve the desired response, a switch to another option from the same first-line group is recommended.

The complete list of recommended agents, sequencing logic, and patient-specific considerations are detailed in the full structured protocol.
