Localized fibrosarcoma classified as high-grade (G2 or G3) carries aggressive tumour biology with an elevated risk for local recurrence and distant metastasis. Management requires a coordinated multimodality strategy, and margin status at surgery is a critical prognostic determinant.
This protocol addresses patients with localized fibrosarcoma graded G2 or G3. The high-grade designation signals significant metastatic potential, and patients in this group are considered candidates for systemic therapy in addition to surgery.
The approach in this setting involves presurgical systemic chemotherapy administered before definitive surgical resection, followed by surgical excision targeting complete tumour clearance. For eligible patients, radiation therapy is incorporated as a further component after surgery.
