Treatment of Localized Fibrosarcoma — Non-High-Grade (G1 or GX)
Clinical Scenario
This protocol addresses patients with localized fibrosarcoma at a non-high-grade histological grade — specifically grade G1 (well-differentiated) or GX (grade not assessable). Surgery is the established standard of care for localized soft tissue sarcomas in this setting.

Primary Goal
Histological tumour-free resection margins (R0)
Achieving an R0 resection is the defining surgical objective to minimise the risk of local recurrence.

- Surgery represents the standard therapy of localized soft tissue sarcomas.
- The surgical procedure depends on the tumour localization, its size and grade of malignancy.
- In case of intramuscular localized soft tissue tumours, the affected muscle compartment should be resected en-bloc as part of the so-called compartment resection.
- If on the other hand the tumours do not reach the muscle origin, and its insertion, or in case of an extracompartmental growth, a wide resection should be performed.
- If possible an R0, tumour margin-free, resection should be achieved in order to minimize the risk of local recurrences.
- Even though a 2 cm margin is sometimes recommended, a generally valid safety margin has not yet been determined.
