Treatment of Femoral Shaft Fracture — Surgical Stabilisation and Malrotation Prevention

Femoral shaft fracture requires prompt surgical management. The critical dual aim is achieving solid bony union and preventing femoral malrotation — a complication that can cause lasting functional deficit if not actively controlled intraoperatively.

Intramedullary nailing is the primary surgical intervention for this fracture. Timing of surgery is a key determinant of outcome — early operative management is strongly favoured. Intraoperative assessment of femoral rotation is a mandatory step during the procedure. Venous thromboembolism prophylaxis is incorporated into the management plan.
