A structured conservative programme is the established first approach for fatigue fracture of the vertebra. When that regimen does not achieve its expected clinical goals, a defined next-line protocol applies.
The initial treatment line included discontinuation of sporting activity, a thoracolumbar orthosis, NSAIDs for pain management, a structured six-level progressive physical therapy programme, and spinal injections in selected cases. That regimen was expected to deliver resolution of low back pain and normalisation of lumbar active range of motion within six weeks. When those targets are not met, escalation is indicated.
For patients who have not responded adequately to conservative care, the next-line protocol involves direct surgical intervention at the fracture site combined with bone grafting. Eligibility is governed by specific criteria on MRI and dynamic imaging that must be satisfied before this approach is pursued.
