Familial adenomatous polyposis (FAP) is associated with intra-abdominal desmoid tumours, which can deteriorate into an acute presentation. When a desmoid becomes symptomatic and imaging demonstrates air-fluid levels alongside generalized peritonitis, the clinical situation demands urgent, structured management.
