Emergency Scenario
Factitious Hyperthyroidism with Sight-Threatening Thyroid Eye Disease (Dysthyroid Optic Neuropathy / Corneal Breakdown)
When factitious hyperthyroidism presents alongside sight-threatening thyroid eye disease — specifically dysthyroid optic neuropathy and/or corneal breakdown — the clinical situation is an emergency. The risk of irreversible sight loss requires immediate, structured management.
Clinical Situation
Emergency: Sight-threatening thyroid eye disease, with dysthyroid optic neuropathy and/or corneal breakdown, carries an immediate risk of permanent sight loss. Prompt intervention is essential.
Treatment Approach
The approach addresses hyperthyroidism with anti-thyroid drug therapy while simultaneously initiating immediate high-dose intravenous corticosteroid treatment under close daily monitoring — the complete regimen, sequencing, and monitoring requirements are in the full protocol.
