  1. For patients not responding to rituximab, the 3rd treatment-line option can be either the use of an immunosuppressant such as azathioprine, cyclosporin or mycophenolate mofetil or yet splenectomy.
  2. The ORR with splenectomy varies from 52% to 100% according to the series from the literature and for ES not responding to rituximab with the sustained activity of both ITP and wAIHA, splenectomy definitely remains a good option.
