Managing Essential tremor when first-choice drug therapy is not enough

Essential tremor is a chronic movement disorder managed through a hierarchy of pharmacological options. When standard first-choice agents prove inadequate or are poorly tolerated, a set of alternative agents — each carrying level B or C evidence — can be considered as monotherapy or add-on therapy.

Clinical scenario

This protocol applies to patients with essential tremor who have not responded adequately to primary first-line agents, or in whom side effects from those agents limit continued use. The goal is to identify the most appropriate next pharmacological step from a broader set of alternatives.

Treatment approach — partial overview

The protocol draws on several pharmacological categories, including agents considered when tremor is significantly worsened by anxiety or emotional stress, as well as other drug classes with varying levels of supporting evidence. The selection, sequencing, and specific considerations for each option — including which patients are appropriate candidates — are defined in the full structured protocol.
