What Is the First-Line Treatment for Essential Tremor?
Selecting the appropriate first-line pharmacological agent for essential tremor is the central clinical decision. The choice between the two recommended options is guided by the patient's concurrent medical conditions and individual tolerability — both carry the highest level of evidence for initial treatment.
Treatment approach
Two agents — a beta-blocker and a second established agent — hold a level A recommendation for first-line therapy and can each be used as the initial choice depending on the clinical context. Treatment is started at a low dose and titrated gradually to optimise tremor control while minimising side effects.
Full titration schedules, dosing strategy, and individualised selection criteria are in the structured protocol below.

- β-Blockers, most commonly propranolol, and primidone are the drugs of choice for treatment of ET.
- Both these agents have level A recommendation, and either can be used for initial treatment of ET, depending on the concurrent medical conditions, and potential side effects.
