Treatment of Esophageal Candidiasis When Oral Therapy Cannot Be Tolerated

Clinical Scenario

This protocol addresses esophageal candidiasis in patients who are unable to tolerate oral antifungal therapy. When the oral route is not feasible, a different therapeutic approach is necessary.

Specific Situation

Esophageal candidiasis with inability to tolerate oral therapy requires an alternative route of antifungal delivery. The management approach in this setting differs from standard oral therapy and follows evidence-based guidance for the intravenous setting.

Treatment Approach

Intravenous antifungal therapy is recommended for patients who cannot take oral medications. This includes intravenous azole therapy and echinocandins as primary options, with an additional less preferred alternative. Once the patient is able to tolerate oral intake, therapy may be transitioned accordingly. Full regimen details, dosing, and sequencing are in the complete protocol.

Clinical Goal

The treatment target is improvement or resolution of esophageal candidiasis symptoms within 7 days of starting therapy.
