Treatment of Advanced or Metastatic Oesophageal Squamous-Cell Carcinoma with PD-L1 CPS ≥ 10

This protocol applies to patients with advanced or metastatic oesophageal squamous-cell carcinoma (SCC) whose tumours show a PD-L1 combined positive score (CPS) of 10 or more — a biomarker-defined subgroup in which specific evidence-based treatment is indicated.

Clinical scenario

Advanced or metastatic oesophageal squamous-cell carcinoma with a PD-L1 combined positive score (CPS) of 10 or more. This CPS threshold identifies a population deriving the greatest benefit from the recommended approach.

Treatment approach: Chemotherapy is central to management in this setting — the specific agent selection, sequencing, and full regimen criteria are detailed in the complete protocol.

PembrolizumabeChT is recommended for advanced, untreated oesophageal SCC. The greatest benefit is seen in patients with a PD-L1 CPS 10 [I, A; ESMO-MCBS v1.1 score: 4; European Medicines Agency (EMA) approval is for tumours with PD-L1 CPS 10, Food and Drug Administration (FDA) approval is irrespective of PD-L1 expression].

ChT with a taxane or irinotecan can be considered in fit patients who have been previously treated with platinum–fluoropyrimidine and/or nivolumab or pembrolizumab [II, B].
