Síndrome de Tolosa-Hunt Sin Respuesta a Terapias Alternativas
Este protocolo aborda el manejo del síndrome de Tolosa-Hunt cuando un ciclo previo de terapias médicas alternativas no ha logrado controlar adecuadamente la afección.
Línea de Tratamiento Previa — Condición de Fracaso
La línea anterior incluyó terapias alternativas:
infliximab
azatioprina
metotrexato
acupuntura
Esa línea no alcanzó los siguientes objetivos, lo que motivó la escalada a este protocolo:
- Resolución de la cefalea y la diplopía
- Recuperación del movimiento de los músculos extraoculares
Enfoque de Siguiente Paso
Cuando las terapias médicas alternativas no han controlado el síndrome de Tolosa-Hunt, este protocolo considera una intervención procedimental dirigida a la región del seno cavernoso.

- Gamma Knife radiosurgery (GKRS) which is a single-fraction radiation therapy offers a very dramatic improvement by applying very low-dose radiation volume into the normal tissues surrounding the target, can be utilized as an alternative treatment option for THS in cases where there is contraindication or intolerability of corticosteroid, although GKRS bears the hazard of late malignancy, the precisity of sub millimetre of GKRS, offers a very dramatic reduction of the low-dose radiation impact into the normal surrounding tissues therefore the risk of secondary on cogenesis is extensively diminished.
