Este protocolo aborda la insuficiencia respiratoria aguda (IRA) que se presenta en el contexto de COVID-19 (infección por SARS-CoV-2). Una proporción de pacientes con COVID-19 desarrolla IRA y síndrome de dificultad respiratoria aguda (SDRA), con riesgo influenciado por la edad del paciente, las comorbilidades, el estado inmunitario y el genotipo viral, entre otros factores.
El manejo se centra en estrategias ventilatorias alineadas con los principios de IRA y SDRA. Para los pacientes no intubados, se incorporan estrategias de posicionamiento específicas en la ruta de atención. El tratamiento farmacológico se orienta por guías clínicas específicas para COVID-19.
A certain proportion of patients with COVID-19 develop ARF and ARDS depending on patient age, comorbidities, immune status, and SARS-CoV-2 virus genotype among other factors.
In patients with COVID-19, ventilatory management is essentially the same as for ARF and ARDS, but awake prone positioning may be considered.
In addition to standard ventilatory management, the benefits of awake prone positioning for non-intubated patients have been posited and examined.
Pharmacological therapies, including corticosteroids, should be administered according to the guidelines and statements specific to COVID-19.
