Next-Line Treatment for Children Under 16 with Myoclonic-Atonic Seizures When Initial Monotherapy Has Not Achieved Seizure Freedom

Clinical scenario

This protocol applies to children younger than 16 years with epilepsy with myoclonic-atonic seizures — a pattern in which a myoclonic seizure occurs and is immediately followed by an atonic seizure. It is the next therapeutic step after single-agent treatment has been trialled without reaching the defined treatment goal.

Escalation trigger — previous line did not achieve its goal

First-line management with antiepileptic monotherapy — using either topiramate or valproic acid — was attempted. The treatment goal of reduction or elimination of seizures, with the aim of seizure freedom, was not achieved, triggering escalation to the next protocol step.

Next-line approach (partial overview)

The protocol advances to combination antiepileptic drug therapy, incorporating two or more indicated agents used together. Which agents are combined, in what order, and under what conditions is specified in the full structured regimen.

Goal: seizure freedom
