Next-Line Treatment for Children Under 16 with Myoclonic-Atonic Seizures When Initial Monotherapy Has Not Achieved Seizure Freedom
Clinical scenario
This protocol applies to children younger than 16 years with epilepsy with myoclonic-atonic seizures — a pattern in which a myoclonic seizure occurs and is immediately followed by an atonic seizure. It is the next therapeutic step after single-agent treatment has been trialled without reaching the defined treatment goal.
Escalation trigger — previous line did not achieve its goal
First-line management with antiepileptic monotherapy — using either topiramate or valproic acid — was attempted. The treatment goal of reduction or elimination of seizures, with the aim of seizure freedom, was not achieved, triggering escalation to the next protocol step.
