Endometriosis with Contraindication to or Intolerance of Estrogen

Clinical Scenario
This protocol is for patients with endometriosis who have a contraindication to or cannot tolerate estrogen, including combined hormonal contraceptives. In this setting, standard estrogen-containing options are not suitable and alternative management is required.
Treatment Goal
The primary clinical objective is relief of endometriosis-related pelvic pain in patients for whom estrogen-based therapy is not an option.
Treatment Approach
The structured protocol outlines management pathways suited to this specific population. For patients in whom medical and conservative surgical options have not achieved adequate control, a definitive surgical intervention may be considered for the treatment of pain.
The complete regimen — including specific interventions, patient selection criteria, and decision sequencing — is available in the full structured protocol.

References

  1. Progestin-only methods (i.e., oral, depot, implant, or hormonal intrauterine systems) are other first-line options that may be considered for those who cannot tolerate or have contraindications to estrogen.
  2. For patients with endometriosis who do not desire future pregnancy and in whom all medical treatments and conservative surgical management options have failed, a hysterectomy with bilateral salpingo-oophorectomy is effective in the treatment of pain.
