This protocol defines the next clinical step when continuous combined hormonal contraceptive therapy — the standard first-line approach — has not achieved adequate relief of endometriosis-related pelvic pain or dysmenorrhea.
First-line empiric therapy with combined hormonal contraceptives (oral pills, transdermal patches, or vaginal rings), used continuously, with or without nonsteroidal anti-inflammatory drugs, was not sufficient.
Second-line management involves agents that act on gonadotropin-releasing hormone (GnRH) pathways. Some approaches incorporate add-back therapy to support tolerability. An alternative agent with a distinct hormonal mechanism is also among the options.
