Endemic Kaposi Sarcoma: Management When Paclitaxel Therapy Has Failed

This protocol addresses the specific clinical situation in which second-line paclitaxel therapy has not achieved the required remission endpoints in a patient with Endemic Kaposi sarcoma. It defines the treatment step considered at this stage of disease progression.

Escalation trigger

Paclitaxel, used as second-line therapy, did not achieve the target outcomes: remission of Kaposi sarcoma with sufficient reduction in tumor size, thickness, and coloration, or alleviation of edema. This protocol represents the next therapeutic step after that failure.

Approach at This Stage

Salvage chemotherapy forms the basis of this protocol. Both oral and intravenous agents are among the options considered — the complete regimen selection, criteria, and clinical algorithm are contained in the full protocol.

As a salvage treatment after anthracycline or paclitaxel therapy, oral etoposide, irinotecan, the previously used ABV regimen (adriamycin or doxorubicin with bleomycin and vincristine) as well as Gemcitabine may also be considered.

In addition to subjective parameters, the size, thickness and coloration of the tumors as well as the presence of edema serve as criteria.
