Emphysematous pyelonephritis occurring in the setting of ureteric obstruction represents a complex presentation in which both the gas-forming renal infection and the obstructed urinary tract must be addressed concurrently.
When ureteric obstruction is identified alongside emphysematous pyelonephritis, relief of that obstruction is a central and urgent management priority. Failure to address the obstructed tract is a recognised determinant of outcome in this condition.
Management involves addressing the obstructed urinary tract through a drainage procedure, combined with systemic resuscitation and antibiotic therapy — except where surgical intervention takes precedence.
