Treatment of Elevated Blood Pressure in Hypertension with Overweight or Obesity (BMI ≥27 kg/m²)

This protocol applies to adults with established hypertension who also have overweight or obesity — specifically a body mass index of 27 kg/m² or higher. Excess weight in this population is a clinically meaningful factor that is directly addressed within the management approach.

The patient presents with hypertension and concurrent overweight or obesity (BMI ≥27 kg/m²). In this group, weight-directed strategies form part of the blood pressure management plan alongside conventional antihypertensive care.

The protocol includes the use of incretin mimetics (a class of weight-management agents) as an adjunct to help lower blood pressure in eligible patients with hypertension and a BMI ≥27 kg/m². In adults with hypertension who also have overweight or obesity with a BMI ≥27 kg/m², incretin mimetics (eg, GLP-1 receptor agonists) when used for weight management may be effective as an adjunct to lower BP.

In adults with hypertension who have obesity with a BMI ≥35.0 kg/m², bariatric surgery (when considered for weight loss) in combination with behavioral interventions and antihypertensive therapies may be effective at lowering BP.
