Treatment of Ebola Virus Disease with Mild to Moderate Dehydration (3–9% Volume Depletion)

In Ebola virus disease, fluid management decisions hinge on the degree of volume depletion. This protocol addresses patients with mild to moderate dehydration — estimated 3–9% volume depletion — who have ongoing fluid losses but remain haemodynamically stable, without shock, and retain the ability to drink.

Clinical Scenario

Patient presents with Ebola virus disease and mild to moderate dehydration (3–9% volume depletion). Ongoing fluid losses are present. There is no shock. The patient is able to drink.

Treatment Approach

Management centres on intravenous fluid replacement, with the choice of solution differing between adults and children. Patients without shock but with ongoing fluid losses (mild-moderate dehydration – see Section 2.4 Fluid status)

For adult patients unable to drink sufficient volumes of liquids to maintain hydration status, it is recommended to give IV fluids.

In adults, it is recommended to alternate between 0.9% saline and Ringer's lactate.

In children, use Ringer's lactate if available but monitor the serum electrolytes regularly.
