Treatment of Vitreous Hemorrhage (With or Without Retinal Detachment) in Eales Disease

Vitreous hemorrhage is a recognized complication of Eales disease and may occur with or without associated retinal detachment. The appropriate management depends on the episode context and the potential for spontaneous resolution.

Approach overview

For the first episode, the management approach is conservative rather than immediately surgical. Specific positional guidance is given to the patient to facilitate natural clearing of the hemorrhage — the complete protocol specifies the measures and the conditions under which a different course is considered.

Clinical goal: resolution of the vitreous hemorrhage, typically expected within a defined time window of weeks.

Vitrectomy is often not needed in the first episode of vitreous hemorrhage.

Cases of nonresolving vitreous hemorrhage with obscuration of central vision of 3 months duration may be subjected to vitrectomy.

Such patients should be advised to keep their head elevated while sleeping.

This permits the hemorrhage to settle down.

The vitreous hemorrhage usually clears between 6 and 8 weeks.
