Primary generalized and primary segmental dystonia represent a clinically important subset of dystonia where symptoms can be widespread or regionally extensive, and where adequate symptom control is not always achieved with conventional approaches.
This protocol applies to patients with confirmed primary generalized dystonia or primary segmental dystonia — forms of dystonia arising without an identifiable secondary cause, involving sustained or intermittent abnormal muscle contractions across broad or regional body distributions.
When medication or other prior measures have not provided adequate improvement in this population, a specific neurosurgical neuromodulation procedure targeting a deep brain structure is supported by strong evidence as the next therapeutic step. Pallidal DBS is considered a good option, particularly for primary generalized or segmental dystonia, after medication or BoNT has failed to provide adequate improvement (level A).
