Treatment of Disseminated Tuberculosis in Children (Under 18) Without CNS Involvement

Disseminated or miliary tuberculosis in a paediatric patient (under 18 years of age) requires a treatment approach that accounts for the patient's age and the extent of disease. The pathway described here is specific to cases where central nervous system tuberculosis has been excluded.

This protocol applies to children under 18 years of age presenting with disseminated or miliary tuberculosis. Confirmation that central nervous system (CNS) disease is absent is a prerequisite for this treatment pathway — if CNS involvement is present, a different protocol applies.

When CNS disease is excluded, a standard 6-month multi-drug regimen is indicated for children in this setting. The regimen is structured in two phases — an intensive initial phase and a shorter continuation phase. Weight-based dosing, the specific agents selected for each phase, and criteria for phase transition are detailed in the full protocol.

For children, if central nervous system (CNS) disease is excluded, give standard 6 months treatment (2HRZ[E]/4HR).
