Disseminated nocardiosis can spread beyond the primary site to involve the central nervous system. When CNS dissemination occurs, cerebral abscess is the most characteristic and serious manifestation, adding significant complexity to management.
This protocol addresses disseminated nocardiosis in patients who have developed central nervous system involvement, specifically presenting with cerebral abscess.
The most common site of dissemination is the central nervous system (CNS), typically manifesting as cerebral abscesses.
Management of this severe presentation involves a structured antibiotic regimen. In select cases refractory to standard therapy, an adjunctive immunomodulatory agent may also be considered as part of the approach.

Adjunctive interferon-gamma has been successfully used in a limited number of patients with severe nocardiosis refractory to standard medical therapy, though this treatment remains unproven outside of patients with chronic granulomatous disease.
