Treatment of Discoid Lupus Erythematosus with Localized Skin Lesions and No Systemic Involvement

This protocol addresses the management of discoid lupus erythematosus presenting with localized skin lesions, where the clinical picture does not include severe or widespread cutaneous involvement, no identified risk of scarring or progression to systemic disease, and the patient is neither pregnant nor breastfeeding.

  • Localized skin lesions of discoid lupus erythematosus
  • No severe or widespread skin involvement
  • No risk of scarring or development of systemic disease
  • Not pregnant; not breastfeeding
Topical therapy forms the foundation of treatment for localized disease. When additional systemic coverage is warranted, the approach involves combining antimalarial agents with a further agent — the specific combination and regimen are detailed in the full protocol.
Full agent selection, sequencing, and any further steps are available in the structured protocol below.

Topical corticosteroids are the mainstay in the treatment of localized CLE being effective in all subtypes.

We recommend MTX up to 20 mg per week as a second-line treatment, primarily in SCLE patients, preferably subcutaneously and in addition to antimalarials.
