Treatment of New-Onset Type 1 Diabetes in Children

Children presenting with newly diagnosed type 1 diabetes require prompt insulin initiation. The regimen must be structured to meet defined glycaemic targets while remaining practical for everyday life.

Management involves subcutaneous insulin therapy combining rapid-acting and basal insulin components, tailored individually to the child — the complete regimen structure, sequencing, and titration guidance are available in the full protocol.
  • A1C target ≤7.5%
  • Fasting / preprandial plasma glucose: 4.0–8.0 mmol/L
  • 2-hour postprandial plasma glucose: 5.0–10.0 mmol/L

Children with new-onset diabetes should be started on boluses of rapid-acting insulin analogues combined with basal insulin (e.g. intermediate-acting insulin or long-acting basal insulin analogue) using an individualized regimen that best addresses the practical issues of daily life.

Children and adolescents <18 years of age should aim for an A1C target ≤7.5%
