Desmoid Tumor of the Abdominal Wall — What Is the Treatment Approach?

This protocol covers the initial management of desmoid tumor arising specifically at the abdominal wall — a site where anatomical location and disease behaviour are central to treatment decisions.

Superficial locations such as the abdominal wall carry particular significance in treatment planning: they represent among the best candidates for surgical consideration in cases of continuous progression, yet the same patients may also be suitable for a careful period of observation first.

Current evidence supports proposing a period of active surveillance — regular clinical assessment and imaging follow-up — as the initial strategy before committing to active treatment. The primary aim during the surveillance period is to confirm tumor stability: stable size on imaging and the absence of growth across consecutive scheduled follow-up assessments.

Superficial sites of disease represent the best indications for surgery in cases of continuous progression, particularly in abdominal wall DT.
However, a period of active surveillance should be proposed as the initial strategy for all patients with DT, before making any decision in favor of active treatment.
