Initial surgery — whether by Mohs micrographic surgery, other forms of peripheral and deep en face margin assessment (PDEMA), or wide excision when PDEMA was not available — did not achieve the target of negative surgical margins. This protocol defines the step taken after that failure.

If initial surgery yields positive margins, re-resection is recommended whenever possible, with the goal of achieving clear margins.
